Allied Community Services, Inc. is Hiring a Service Coordinator (Enfield, Connecticut; Pearson Way) Near Enfield, CT

Reportability Reports directly to the Department Director as assigned.Supervisory Responsibilities None.Essential Functions/Specific Physical TasksTo perform this job successfully, an individual must be able to perform each essential function satisfactorily. The work environment characteristics described are representative of those an employee encounters while performing the essential functions of this job. Reasonable accommodation may be made to enable individuals with disabilities to perform essential functions.Work environment: Work primarily indoors, in moderate (68 – 70) temperatures, for up to an entire eight-hour shift. May work outdoors, in cold or hot environments, traveling to and from various work sites. May work in noisy areas, with unusual lighting, 0 – 5 times per week. Machinery used may include lift tables, weighing scales, letter openers, computer equipment. May work with irritant chemicals (cleaning products) or small appliances.General physical mobility: Sitting – one hour per day, may be sporadic opportunities throughout an eight hour shift; up to four hours during a training. Standing – up to an entire eight hour shift, usually in 4 – 6 hour increments, sit to stand 5 – 10 times per day. Walking – constant throughout work area on a cement floor, to and from supplies, to and from office area, in and out of the building. Climbing stairs – by the loading dock, 1 – 2 times per month. Bending, stooping, twisting of the trunk – up to 25 times per activity per day. Reaching, pushing, pulling – up to 65 pounds, up to 50 times per day. Typing or keyboarding – up to 6 hours per day with opportunity stretch as needed.Weight handling requirements: Lifting or carrying up to 65 pounds, up to 30 times per day; Strength: HEAVY.Personal requirements: Utilizing specific knowledge/education experience of behavioral intervention, safety procedures, the ability to deal with unusual and bizarre behavior without immediately considering it a crisis situation; utilizing good communication skills, ability to work in a fast-paced, demanding atmosphere, and operating a vehicle. A high degree of both visual efficiency and the ability to receive and relay detailed information is also required.Response to crises: Maximum – work in high-stress, or may encounter emergency, situations 0 – 10 times per day. Duties
  • Comply with participant rights policies, quality assurance standards, and Allied policies and procedures.
  • Coordinate with agency quality assurance personnel to ensure state and federal data compliance.
  • Works collaboratively with the Department Supervisors, Service Coordinators, Director, Staff, and Stakeholders to accomplish program and individualized goals and objectives.
  • Compiles data, analyzes individualized programming and recommends programs and services for assigned participants.
  • Writes and/or assures that report documentation is maintained in all assigned areas, including, but not limited to, the following:
    • Attendance Reporting
    • Incident and Accident reports
    • Guidelines, Protocols, and Medication Orders
    • Program plans/IPs, vocational assessments
    • Transition, discharge, admission summaries
    • Uploading and maintaining documents on Therap.
    • Maintaining Therap programs, individual support plans, and personal information.
  • Prepares and submits written reports to the QA manager for review and approval.
  • Participates in and presents reports on individual progress to interdisciplinary teams.
  • Advocates and recommends programming choices based on the needs of each individual in collaboration with the IDT.
  • Provides management of program participant documentation.
  • Trains staff in implementing individuals’ programs, goals, and objectives.
  • Applies concepts of human rights, participant rights, and Agency policy regarding abuse and liability issues in daily practices.
  • Is knowledgeable in and applies emergency intervention techniques (medical, behavioral environmental), task analysis, community integration, behavior modification principles, Agency goals and objectives; Gentle Teaching/Try Another Way methodologies.
  • Serves as a consumer role model and follows chain of command in daily procedures/processes/events. Practices Allied’s Values of Excellence, Respect, Integrity, Innovation, Diversity, and Collaboration in everyday practice.
  • Provides coverage for staff shortages and supervisor absence.
  • Maintains all required training and certifications.
  • Responsible for reading and becoming familiar with the staff Personnel Handbook and all other related Agency Policies and Procedures.
Qualifications
  • Proficient in computer skills: has a good understanding of computer systems and can use them effectively to complete tasks.
  • Ability to identify and analyze problems, develop solutions, and implement them professionally.
  • Strong analytical skills to analyze data, identify patterns, and draw conclusions from them.
  • Ability to troubleshoot: has the ability to identify and resolve technical issues in a timely and efficient manner.
  • Proficient in software applications: has experience working with various software applications (Therap, Word, Microsoft Office) and can use them effectively to complete tasks.
  • Motivation and commitment to improve the lives of persons with disabilities.
  • Ability to write reports, draw graphs, compute basic statistical data, and perform general computer-entry tasks.
  • Ability to organize and prioritize projects and manage multiple deadlines.
  • Ability to communicate effectively and work productively with consumers, service providers, community members, and outside agencies through verbal and email correspondence.
  • Demonstrates effective soft skills, such as emotional intelligence, empathy, and effective communication.
  • Maintaining a professional demeanor even in high-stress situations.
  • Advanced knowledge of service systems designed for persons with intellectual disabilities.
  • Ability to work productively both interdepartmental and interagency.
  • Ability to deal with a wide variety of people as the Agency’s representative.
  • BA plus minimum 2 years providing direct service to individuals with intellectual disabilities; experience may substitute for education.
